We are seeking a Providence California Regional Services who will be responsible for the development and implementation of all aspects of the Active Aging Program, including recruitment, training and management of program volunteers; recruitment, enrollment, fitness testing and maintenance of program participants; development of collaborative relationships within the community; budget tracking; and program evaluation.

In this position you will:

  • Participant Recruitment, Enrollment, and Oversight:

  • Recruit new program participants through marketing efforts and outreach events with a primary focus on community groups. Plans and conducts orientation meetings as needed

  • Conduct one-on-one and group enrollment meetings, completing all enrollment paperwork and preparing folder for Support Buddy assignment. Conduct individual Senior Fitness Test on each enrollee upon enrollment and sends results and recommendations. Enter and maintains database of participants. Provide enrollees with individually-tailored resources

  • Respond to all phone calls

  • Call enrollees to check on progress with Support Buddy and program and to offer information and support. Follow up with Support Buddies as needed

  • Facilitate use of program’s Access Fund for those needing financial assistance

  • Wellness Programs:

  • Plan, implement and evaluate physical activity related wellness programs and exercise classes at variety of community sites in collaboration with Senior Services and community agencies

  • Develop ongoing “Just Try It” series for participants in collaboration with community sites

  • Plan and implements annual National Senior Health and Fitness Day event in collaboration with community sites

  • Facilitate physical activity discussion groups for program participants at community sites

  • Prepare flyers for all Wellness Series Programs

  • Collaborative/Community Projects:

  • Participate in the development of collaborative community-based projects and activities that improve access to healthcare and promote physical, mental and social health, within the context of the Active Aging Program

  • In defining unmet community needs and planning for programs to address those needs, utilizes an assets mapping/capacity-building approach that capitalizes on the skills of those involved, including volunteers

  • Program Evaluation:

  • Monitor program progress compared to goals and objectives

  • Conduct Senior Fitness Tests upon enrollment and every six months for participants. Create and analyze reports from Senior Fitness Test software

  • Survey members to track progression through Stages of Readiness to Change

  • Use evaluation forms at wellness programs and summarizes information for reference

  • Make recommendations for program improvement based on evaluation

  • Report program progress, including participation in draft of progress reports to funding agency, as required

  • Documentation/Information Management:

  • Use Meditech and Microsoft Office programs for communications, word processing, file creation and maintenance, and other uses for program needs in a timely manner

  • Use Senior Fitness Test software to document test scores for program participants. Updates database in a timely manner

  • Prepare required reports for Department Manager

  • Adhere to budget and reports expenses to Department Manager for budget tracking

  • Leadership/Supervision:

  • Volunteers:

  • Recruit, screen, train and supervise AAP volunteers

  • Establish and maintain an effective working relationship with volunteers, addressing any volunteer issues or problems in a timely manner

  • Prepare for and conduct regular volunteer staff meetings and ongoing training. Plans appropriate recognition events

  • Maintain volunteer files and record in compliance with policies and procedures

  • Self:

  • Develop, maintain and demonstrate up-to-date knowledge on issues related to physical activity and aging, related resources, community health and other aging issues by attending seminars, trainings, or conferences, reading materials or other methods. Maintains current education record

  • Increase visibility of Active Aging Program and other Senior Services/Community Health programs via community networking groups, community functions, speaking engagements and other professional activities, as agreed upon by Department Manager


Required qualifications for this position include:

  • Bachelor's Degree in Physical Education, Exercise Physiology or related field

  • Must be CPR certified

  • This position may require the employee to operate a hospital-owned vehicle that has been assigned to the department. Per the organization’s “Vehicle Use and Insurance Coverage” and “Driver DMV Record Checks” policies, the employee may have to:

  • Possess a valid California driver’s license, appropriate to the vehicle type and operation/use. Employee if to provide the Department Manager with a current Driver’s License (at the time of hire and renewal) to be maintained in his/her department personnel file

  • 2 years experience in physical education or related field

  • Bi-lingual (Spanish) required

  • Excellent interpersonal skills

  • Knowledge of physical activity-related issues of older adults

  • Ability to work with a variety of individuals and groups, e.g., peers, older adults, community agency personnel, physicians, etc.

  • This position may require the employee to operate a hospital-owned vehicle that has been assigned to the department. Per the organization’s “Vehicle Use and Insurance Coverage” and “Driver DMV Record Checks” policies, the employee may have to:

  • Understand that a DMV driving record check will be conducted for the past three years and will be obtained in January and July of each year

  • Comply with all laws, rules, and regulations governing the operation of a motor vehicle. Traffic violation citations are the responsibility of the employee driver and are to be paid in a timely manner

  • Understand that appropriate action may be taken based on the DMV points as outlined in the “Driver DMV Record Checks” policy and procedure

  • Immediately report any motor vehicle accident to his/her Manager or Supervisor

  • Be familiar with and follow the “Vehicle Use and Insurance Coverage” and “Driver DMV Record Checks” policies

Preferred qualifications for this position include:

  • Experience working with older adults on an individual and group basis

  • Teaching experience

  • Program coordination experience

  • Supervision of volunteers
