Description:

Providence is calling a Clinical Social Worker MFT MS, Non-Licensed (Per Diem/Variable Shift) to Providence Little Company Of Mary San Pedro Hospital in San Pedro, CA.

We are seeking a Clinical Social Worker MFT MS, Non-Licensed who will provide clinical social work services on the Crisis Stabilization Unit (CSU) in a manner consistent with the philosophy and objectives of LCM-SPH and the NASW. The Social Worker is responsible for psychosocial assessments. Participate in treatment team planning, processing commitments, discharge planning/case management service, crisis intervention, and liaison services with referral sources and other departments within the Providence Health & Services and community. Patients to include adults 18 years or older.

In this position you will:

  • Provide age appropriate social service intervention proactively and upon request to individual patients, couples, families and groups to include: Psychosocial assessments, short term psychotherapy, supportive therapy, coordination or care, discharge planning and community resource referrals

  • Review charts and completes intervention with emphasis on identification of issues related to psychosocial assessment, treatment plan, intervention/referral and follow-up

  • Actively participate/facilitate patient care rounds/case conferences

  • Proactively identify unmet patient needs in area of service and initiates programs/services/in-services as needed. Proactively identify areas of unmet need in relation to patient rights and follows through with appropriate intervention

  • Knowledgeable and helpful in assisting patients with advanced directives. Is current on ethical issues in patient care areas

  • Collaborate with other department staff relative to shared cases in a timely manner

  • Maintain knowledge of policies related to clinical and organizational ethics

  • Maintain up-to-date knowledge of reporting responsibilities and shares with healthcare team evidenced by providing employee in-services as the need is identified. Maintain knowledge of policies related to clinical and organizational ethics

  • Attend the majority of staff meetings and participates in departmental functions. Communicate when unable to attend

  • Accept on-call and/or weekend assignments as requested or required by the department

  • Concentrate on area of assignment but maintain the flexibility to perform routine duties in any area of the hospital

  • Assist the department in special projects, as assigned.

  • Assist with social work coverage within other areas of the ministry including inpatient, emergency department and post-acute care services.

  • Documentation/Information Management:

  • Demonstrates working knowledge of the hospital Information System.

  • Practices professional confidentiality and serves as a role model for others.

  • Documents appropriate information pertaining to the individual patient in a specific, accurate and thorough manner to allow for follow-up problem/resolution.

  • Maintains proficiency in computer and written documentation as defined by department guidelines.

  • Keeps an updated personnel record per facility policy (i.e., PPD, Educational Record).

  • Leadership and Supervision:

  • Practices in a professional and knowledgeable manner.

  • Serves as an internal and LCMSA resource for others regarding psychosocial issues in patient care.

  • Acts a role model and maintains a positive approach to change.

  • Practices effective two-way communication.

  • Provides referrals for psychosocial issues to healthcare team members (i.e., referrals to EAP).

  • Demonstrates the ability to make appropriate decisions within the scope of responsibility.

  • Shares valuable information with co-workers and peers.

  • Development:

  • Completes mandatory continuing education requirements for re-licensure, as appropriate, independently.

  • Demonstrates interest in professional growth through attendance of workshops and seminars.

  • Attends LCMSA in-services and other educational activities to maintain current knowledge of practice and technology that is utilized in the role.

  • Provides in-services to others on the healthcare team (i.e., Speakers Bureau).

  • Provides orientation to new employees/students, as requested.

  • Provides clinical consultation to staff/physicians regarding the psychosocial aspects of patient care, assisting others to improve their knowledge base.

  • Sets valuable and attainable annual objectives.

  • Performance Improvement:

  • Upholds and strives to meet the Continuum/System goals and objectives - accepting change positively.

  • Participates performance improvement in patient care related issues using the C.A.R.E. methodology.

  • Identifies issues, seeks resolution and ensures follow through.

Qualifications:

Required qualifications for this position include:

  • Master's Degree in Social Work, Psychology, or Marriage and Family Therapy from an accredited program

  • Pursuing licensure and registered with the California Board of Behavioral Sciences (BBS) as an Associate Clinical Social Worker (ASW) or as a MFT Intern (MFTI) required

  • CPI certification (nonviolent crisis intervention training) within 1 year of hire

  • 2 years experience in healthcare related setting with psychiatric mental health patient population or within an emergency department working with psychiatric patients

  • Requires advanced communication skills to communicate effectively with patients, interdisciplinary team physicians and the community

  • Knowledgeable of case management, group leadership skills, age specific growth and development, crisis and behavior management, family therapy theory and practice, and community resources with discharge planning experience preferred

  • Requires flexibility and strong self-motivation / organizational skills to prioritize work and make decisions with minimal supervision

  • Must be able to communicate verbally and in writing; physical agility to manage patients; push, pull and lift up to 40 pounds; intermittent sitting, standing, walking, bending and stooping; drug and alcohol free as evidenced by negative results of post-offer, pre-employment urine drug screen

  • Requires a courteous and cooperative service-oriented demeanor

  • Computer experience/literacy

  • Bilingual in Spanish
