We are seeking a Activities Aide who will assess the needs and interests of all residents at Providence Little Co of Mary Subacute Care Center. The Activities Specialist will provide patient care to the following age groups; adolescent, adult, and adult geriatric. The activity program, in compliance with state and federal regulations, is provided for each individual to meet the assessed needs and interests. The resident would be enabled through participation in the activity program to maintain his/her highest attainable social, physical, emotional and spiritual functioning.
In this position you will:
Documentation and Intervention:
Interview residents and/or appropriate significant others to obtain information on recreational history, individual preferences, general physical and cognitive abilities, current needs and interests and documents findings in the EMR
Provide a plan of activities for every resident based on individual preferences, abilities, needs and interests. Provides opportunities for empowerment, restorative, maintenance and supportive activities as determined during the evaluation
Meet and greet new residents to educate, inform and encourage residents, families and interested others to participate in available programing
Post monthly calendars and provide information regarding available activity supplies and services
Lead meaningful small and large group activities according to the planned activity schedule
Appropriate activity programs must begin on time according to the posted scheduled
Residents, families and interested others are able to participate as appropriate
Residents are encouraged to participate up to their attainable capacity and are encouraged to do so by the activity aide
Assist in the development of activity planning
Provide meaningful individual programs for residents unable or uninterested in attending group activities (i.e.: isolation, bed bound, confused or disruptive etc.)
Individual programs and/or monitoring are provided at least 3x per week or according to the residents’ plan of care
Sensory stimulation, reality orientation and/or validation is provided to the cognitively impaired residents as appropriate
Activity supplies are available to residents and visitors for independent use while avoiding excessive costs
Assist residents with recreation equipment as needed (music, computer, IPAD etc.)
Document patient response to planned activities in relation to the stated goals on the interdisciplinary treatment plan and EMR as appropriate. Interventions and their effects on patient care are documented on a daily basis, with type of treatment, date and time spent indicated
Safely transport residents and necessary medical equipment to and from activity programs and outings
Assist with planning and implementation of holiday observances and special events
Participate in Interdisciplinary Care Plan meetings and/or communicate pertinent information regarding resident care to other care providers while maintaining confidentiality and respect for the resident
Intervention:
Demonstrate ability to apply planned therapeutic activities and safely progress patients through the levels of treatment within the maximum capabilities of the patient
Provide planned activities consistent with the patient’s total rehabilitation program
Provide goal directed educational activities to patients/families based on the interdisciplinary treatment plan
Provide community based planned activities as necessary to facilitate functional community reintegration
Develop and coordinate community reintegration activities in conjunction with OT, PT, and Speech
Develop a monthly calendar of recreational/leisure group activities
Provide individualized and group treatment services designed to improve patient’s psychosocial, cognitive, and physical abilities as necessary pre-requisites to future leisure/social involvement
Dispense resource information handouts to patients/families for educational purposes upon requires of the patient or family
Communication:
Develop and maintain effective communication with the patient, family and rehabilitation team members
Participate in team and family conferences as requested to discuss patient goals, problems and ongoing discharge plans
Participate in problem solving conflicts, i.e., overlapping responsibilities, scheduling problems
Consult with other members of the rehabilitation team to coordinate therapeutic activities
Establish and maintain a therapeutic relationship with patients; use praise and other reinforcers to encourage appropriate behavior
Demonstrate the ability to communicate with the patients and families in establishing favorable attitudes and motivation toward the patient’s rehabilitation process
Informally confer with other members of the rehabilitation team; incorporat positive suggestions and relevant information into the rehabilitation program
Stewardship:
Appropriately utilize petty cash as needed for miscellaneous items
Appropriately utilize Dietary requests for special events and adheres to specified budgeted amount
Use time effectively and ensures adherence to daily schedule
Maintain daily productivity standard and informs supervisors of any changes in daily productivity
Adjust personal schedule in case of unit crises to meet department need
Demonstrate “flexibility” for cross training between programs
Qualifications:
Required qualifications:
Completed a thirty six (36) hour Activities Director Certification Course
Experience in basic computer skills
Knowledge of basic computer skills
Ability to establish and maintain a courteous and cooperative (service-oriented) working relationship with patients, families, and co-workers
