We are seeking a Patient Health Advocat e who will coordinate care for high risk, high utilizing patients. The Health Advocate supports the quadruple aim for population health management as an extension of the trusted Primary Care Provider to promote the patient centered medical home. The Health Advocate is part of a larger Care Management team. Primary focus areas for the Health Advocate is evaluating for social determinants of health, identifying alternate sites of care, enhancing patient access to urgently needed services, ensuring that patients understand their care plans, facilitating care outreach of transitions of care, and in partnership with ambulatory Care Managers, longitudinally following high risk patients to help reduce the burden of care. Assists in closing care gaps and identifying barriers to care. Provide patients with internal and external resources to help coordinate care.
In this position you will:
Assist in patient care management by identifying high risk patients in need of ongoing care management and support Providers and Care Managers with urgent care coordination needs
Actively participate in the planning, preparation and data collection on high risk patients. Anticipate the needs of assigned patient population
Promote successful program implementation and quality cost-effective outcomes within the scope of responsibility. Maintain technical and behavioral competency requirements within scope of practice. Demonstrate initiative for self-improvement and complete required competencies
Participate as a team member and communicates effectively with the health care team. Participate in staff meetings and other interdisciplinary meetings
Identify internal and external resources and connects patients to resources for effective care coordination. Assist patients in navigating complex health systems and insurance plan coverage
Perform transition of care calls in a timely manner to prevent readmissions and ED visits
Review reports/lists of rising risk and high risk patients with care gaps and intervenes in a timely manner to prevent further decompensation and utilization. Escalate complex clinical cases to Providers and Nurse/Social Work Care Managers for clinical management
Demonstrate excellent communication skills. Communicate with patient/family in a clear, concise, and understandable manner. Ensure patients and caregivers understand the patient's care plan and utilizes techniques such as motivational interviewing to help prioritize and support the patient's goals. Obtain confirmation that information given to patient/family was understood
Help to facilitate appointment scheduling, test/surgeries, and coordination of care. Ensure patient follow-through of urgent/important appointments and works to remove barriers for non-compliance
Document patient's concerns and symptoms, reactions, and change (appropriate to age) in the electronic medical record. Relay and escalate information to Providers, Care Managers, and other care team members in an accurate, concise, and timely fashion, ensuring follow-through by clinically licensed staff on symptom-based issues
Follow-up with patients/caregivers, ensuring that the patient/caregiver receives care plan updates, results of laboratory and diagnostic tests, special test instructions, and other information in a timely manner
Coordinate communication and transactions between patient, ancillary services, consultants, and physicians
Coordinate authorizations required by insurance
Create the necessary care management reports timely and accurately
Work with Utilization Management and insurance companies, as necessary, to assist patient
Assist in closing care gaps and Identifying barriers to care

Demonstrate competence in assessing and interpreting age appropriate data about the patient's status in order to identify age-specific needs and provide the care needed
Additional Requirements:
Demonstrate the appropriate communication skills for the patient population served. Demonstrate knowledge of age-specific community resources
Information Management:
Treat all information and data within the scope of the position with complete confidentiality and security. Maintain all practices to protect PHI and follows to HIPAA guidelines
Risk Management:
Cooperate fully in risk management activities and investigations
Safety Requirements:
Is knowledgeable of, observes and complies with Codes of Safe Practice, safety policies and emergency procedures
Qualifications:
Required qualifications for this position include:
H.S. Diploma or GED
Graduate of a State recognized vocational school or Medical Terminology coursework: Working knowledge of Medical Terminology
Current BLS (Basic Life Support) for Healthcare Providers – American Heart Association (AHA)
5 years related work experience in an acute care or ambulatory care facility and/or medical group practice
Basic computer skills in a Windows operating environment including, Microsoft Word and ability to learn various new computer systems and software
Excellent communication skills
Has completed coursework or has working knowledge of medical terminology
Knowledge of sanitation, personal hygiene and basic health/safety precautions applicable for work in a clinic setting. Knowledge of infection control procedures and safety precautions
Knowledge of computer systems and software used in functional area
Ability to speak and write effectively at a level appropriate for the job
Ability to solve problems and identify solutions
High-level customer service skills in interactions with all patients, families, and staff, including high volume and stressful situations
Ability to plan, organize, prioritize, work independently and meet deadlines
Ability to work as a contributing member of an interdisciplinary team
Ability and willingness to travel between Ministry Locations
Preferred qualifications for this position include:
Associate's Degree in a related field
Bachelor's Degree in a related field
MA certificate
Current LVN license in State of California
Managed care experience
EPIC, Allscripts Enterprise, and/or IDX experience highly desirable
Proficient in all Microsoft Office applications including Word and Excel
Bilingual English/Spanish communication skills
